Job Title: Intake Coordinator

Work Model: Hybrid

Work Hours: Monday–Friday, 7/8 PM to 3/4 AM

M&N Home Care Services is a respected organization committed to delivering high-quality home care services to those in need. As a rapidly expanding agency, we are seeking enthusiastic and dedicated candidates to join our team as an Intake Coordinator.

Responsibilities:

As an Intake Coordinator, your responsibilities will include (but are not limited to):

  • Receiving and processing referrals from clients, healthcare professionals, and partner agencies.
  • Assessing potential clients' eligibility and requirements.
  • Coordinating with the scheduling team to ensure timely provision of home care services.
  • Maintaining accurate, up-to-date records of clients and services provided.
  • Communicating with clients and their families to address queries or concerns.
  • Collaborating with the HR team to onboard and train new caregivers.
  • Assisting in organizing workshops and training sessions for caregivers and clients.

Qualifications:

  • Bachelor's degree in any discipline from a reputed university. Freshers are encouraged to apply.
  • Knowledge of healthcare environments, clinical terminology, and health information systems strongly preferred.
  • Excellent organizational and time management skills, people skills, verbal, and written communication skills required.
  • Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendars, and email.
  • Health Home experience is a plus.
